Healthcare Navigation

Let us handle the complicated parts of healthcare for you.

When facing a health issue, having to find affordable in network providers, schedule appointments, and review bills and insurance claims for accuracy can be confusing and time-consuming; an additional stressor during an already difficult time. We offer services to address the four main financial and administrative challenges in healthcare.

Coordinated Care

Continuity MD’s Healthcare Navigators can help manage patient care, allowing your employees to focus on staying productive. Navigators can:
  • Assist in finding referrals if a patient’s care needs to be escalated
  • Coordinate prescription refills between the provider and pharmacy
  • Help transfer medical records from one provider to another

Bill Advocacy

Bills from a provider can be frustrating and hard to understand, and dealing with them can require patience and knowledge. When needed our Healthcare Navigators can:

  • Verify the accuracy of large and complex bills
  • Identify billing discrepancies and other clerical errors
  • Confirm if services are eligible for FSA reimbursement
  • Advocate for bill reductions when appropriate
  • Submit out-of-network insurance claims and claims appeals
  • Find savings on prescription medications

Employees without health insurance

Our Healthcare Navigators help find to the most affordable healthcare options that stretch your hard earned dollars to meet your medical needs.

  • We collaborate with medical providers to offer an affordable DPC model that rivals traditional health insurance plans but at a fraction of the cost.
  • Our Healthcare Navigators can search for affordable medical services when in unfamiliar locations.

Insurance Support

For employees with health insurance, our Healthcare Navigators bridge the gap between insurance companies and healthcare providers. They can help you deal with — and better understand — concerns surrounding:

  • Plan deductibles and copays
  • Covered services, providers, and facilities

To reduce ER visits.

Without access to a doctor’s office, people who are ailing often head for the nearest emergency room.. Studies have shown a 25 percent reduction in ER visits among patients whose primary-care providers offered extended hours. For children the data are even more dramatic: The Journal of Pediatrics reported that pediatricians with evening hours reduced their young patients’ ER visits by 50%.

To reduce hospitalization and improve health.

Patients who consult with their regular physician or group tend to require less time in hospitals. The Journal of General Internal Medicine reported that care “outside usual business hours … is associated with improved patient outcomes.”

To deliver personalized care and create patient satisfaction.

The Annals of Family Medicine views evening and weekend appointment options as key to a patient-centered society. When patients are sick, they want to see someone who knows them on a personal level and is looking out for their best interest.
